29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about November 12, 2020, at a residential framing construction jobsite, employees working without hardhats from lower levels, while assisting framing employees working at upper levels, were exposed to head injuries from falling objects and overhead struck-by hazards.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about November 12, 2020, and at times prior thereto, at a residential framing job site, employees using pneumatic nail guns and circular saws were not provided with eye protection. The employees were exposed to eye injuries by flying objects.

29 CFR  1926.1051(a):Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  On or about November 12, 2020, and at times prior thereto, at a residential framing jobsite, the employer did not provide stairways or ladders for framing employees to access higher levels of the construction project. Employees were exposed to fall hazards of 8 to 16 feet to the ground below.

On or about November 12, 2020, and at times prior thereto, at a residential framing jobsite, the employer did not provide a training program to employees who accessed upper levels without stairways or ladders.  Employees were not trained to understand, have the knowledge to recognize and how to avoid ladder hazard.
