29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) On or about November 12, 2020 and times prior thereto at the jobsite employees were performing roofing operations and were exposed to the hazard of 22-feet to the ground below.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) On or about November 12, 2020, employees performing roofing operations on the front side of the residential home utilized a portable extension ladder was used to access the upper landing surface of a residential home under construction. Employees were exposed to the hazard of falling 22-feet to the ground below;  (b) On or about November 12, 2020, employees performing roofing operations on the backside of the residential home utilized a portable extension ladder was used to access the upper landing surface of a residential home under construction. Employees were exposed to the hazard of falling 11-feet to the ground below.
