29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At worksite located at 15320 Meadows at Creekside PARKVILLE, MO 64152 -- Employee was using extension cord to power a jobsite portable skillsaw. The cord had obvious strain relief. This exposed the employee to electrical shock and/or burns. This was documented on or about 11/12/2020.

The employee was exposed to fall hazards. This was documented on or about 11/12/2020.
