29 CFR  1926.102(a)(2):The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.     On or about November 18, 2020,  at a construction site located at 2295 Woodforest Parkway North, Montgomery, TX,  an employee was exposed to eye-injuries when using a DeWalt  saw to cut decorative stones without eye protection.

29 CFR  1926.451(b)(2)(ii): When walkways between scaffold sections are less than 18 inches wide, employees were not protected from falls by guardrail systems or personal fall arrest systems:   On or about November 18, 2020,   at a construction site located at 2295 Woodforest Parkway North, Montgomery, TX,  employees were exposed to fall hazards when using a 2 X 6 wooden board  as a scaffold without guardrails or personal fall arrest system.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall protection.      On or about November 18, 2020,   at a construction site located at 2295 Woodforest Parkway North, Montgomery, TX,   employees were exposed to fall hazards while  installing stone near the edge of a canopy that was six feet or more above a lower level.
