29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Job site, 1706 Termon Avenue, PIttsburgh, PA: On or about November 19, 2020, during roofing work, the employer did not ensure that the employees working in areas below where other employees were working above them wore protective head wear where exposed to objects that could fall or fly onto their heads.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  Instances including, but not limited to, the following:  a) Job site, 1706 Termon Avenue, PIttsburgh, PA: On or about November 19, 2020, during roofing work, the employer did not ensure that the employees working on the porch roof had appropriate fall protection systems/devices where exposed to a fall hazard of up to approximately 15 feet.
