29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On the south side of the construction site, the employer did not provide a scaffold ladder where employees could access and egress the tubular welded frame scaffold safely, exposing them to fall hazards.

29 CFR  1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.   On the south side of the construction site, an employee used the second tier of a tubular welded frame scaffold without fall protection to access the roof area to perform masonry work.  The employee was expose to a fall hazard of approximately 12-feet to the ground below.
