29 CFR  1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  At jobsite located at 2587 Dorris Wichita KS,  the employer did not ensure that appropriate permission from the manufacturer was obtained by the user prior to using an attachment on the Skytrack Telehandler, and did not ensure that markings for the capacity, operation and maintenance were changed accordingly. This condition exposes employees to struck-by and crushing hazards.
