29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) On or about 11/19/2020, at the maintenance shop of FineLine Pipeline Inc., located on 5998 Rowland Road, in Conesus, New York, the employer did not have a Lockout/Tagout (LO/TO) program with specific procedures and training for employees performing maintenance activities such as, but not limited to: maintenance of tractor-trailer trucks. Employees were exposed to crushing hazards when performing a brake system maintenance under a Peterbilt tractor and Trailstar trailer truck. Unexpected movement of the tractor-trailer was not controlled by steps that may include but are not limited to: parking the vehicle on a level surface, keeping the trailer bed down, engaging the brakes on the tractor and trailer, turning off the engine, securing the ignition keys and chocking the wheels. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 11/19/2020, at FineLine Pipeline Inc., located on 5998 Rowland Road, in Conesus, New York, the employer did not develop lockout/tagout procedures for employees to utilize while performing maintenance activities on tractor-trailer trucks. Employees performing a brake system maintenance on a Peterbilt tractor and Trailstar trailer were exposed to crushing hazards as a result of not controlling the unexpected movement of the tractor-trailer. Steps such as, but not limited to: parking the vehicle on a level surface, keeping the trailer bed down, engaging the brakes on the tractor-trailer, turning off the engine, securing the keys and chocking the wheels were not taken. ABATEMENT DOCUMENTATION REQUIRED
