1926.451(c)(2)(v)  Fork-lifts were used to support scaffold platforms in which the entire scaffold platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  This violation occurred on or about November 30, 2020, where employee(s) used an elevated work platform attached to Genie all terrain forklift to access the roof.  The platform was not secured to the forks and did not have fall protection exposing employee(s) to a fall of more than 20 feet to the ground below.�

29 CFR  1926.451(g)(1):  The employer did not ensure that each employee on a scaffold more than 10 feet above a lower level was protected from falling to that lower level.   This violation occurred on November 30, 2020, where employees used an elevated work platform attached to Genie aerial lift to access the roof.  Employees were exposed to a fall of approximately 20 feet to the ground below.
