Klopfenstein Art Equipment Manufacturing, LLC - Mansfield, Ohio: On or about November 11, 2020, the employer did not develop and implement an energy control program consisting of training, procedures, and audits, to protect employees from hazardous mechanical and hydraulic energy sources.  Employees that performed set-up tasks such as, but not limited to, die changes on the brake press and blade changes on the automatic saw were exposed to amputation and caught-in injuries while servicing the machines.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Employees were exposed to caught-by hazards from the unguarded chuck and the rotating bit when wearing gloves during operation.  b. Klopfenstein Art Equipment Manufacturing, LLC - Mansfield, Ohio: On or about November 11, 2020, employees that operated the Dutch Saws automatic saw (s/n 800892) were exposed to amputation and caught-by hazards.  The machine was not adequately guarded in that the guarding devices were bypassed and did not prevent from accessing hazard areas of the machine while it was in operation.  c. Klopfenstein Art Equipment Manufacturing, LLC - Mansfield, Ohio: On or about December 2, 2020, the employer did not ensure that the hold-down clamps of the Niagara Power Shears (s/n 59480) were guarded to protect employees from amputation injury.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
