29 CFR  1910.134(e)(2)(ii): The medical evaluation shall obtain the information requested by the questionnaire in Sections 1 and 2, Part A of Appendix C of this section.   a. OhioHealth Rehabilitation Hospital, LLC. located at 1087 Dennison Ave, Columbus, OH 43201:   On or about December 28, 2020, the medical evaluation provided to employees to complete did not contain all of the information from Sections 1 and 2, Part A of Appendix C of the Respiratory Protection Standard. The employer required employees to wear N95 respirators while providing care to suspected and confirmed positive COVID-19 patients.
