29 CFR  1926.452(j)(1): Pump jack brackets, braces, and accessories were not fabricated from metal plates and angles. Each pump jack bracket did not have two positive gripping mechanisms to prevent any failure or slippage.   a) On December 3, 2020 at the worksite, where the top of the pump jack scaffold was secured with wood exposing employees to a fall hazard when installing siding.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:  a) On December 3, 2020 at the worksite, where employees were exposed to a fall hazard when installing siding from a pump jack scaffold without the poles secured to the structure at the bottom.
