29 CFR  1910.1200(e)(1):  Hazard communication - 1910.1200.  The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) Throughout the facility, on December 3, 2020 and at times prior to, the employer did not develop or implement a written hazard communication program for employees who used hazardous chemicals including but not limited to diesel fuel, hydraulic fluid, and propane, resulting in a chemical hazard.

29 CFR  1928.57(b)(2)(ii):Revolving shafts, including projections such as bolts, keys, or set screws, except smooth shaft ends protruding less than one-half the outside diameter of the shaft and its locking means, were not guarded.  (a) In the facility yard, on or about November 30, 2020 and at times prior to, the revolving power take-off shaft associated with the Van's sprayer trailer had an unguarded 2.5 to 3 inch set screw (bolt) extending from the shaft exposing employees to caught-in hazards.
