29 CFR  OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees by exposing employees to struck-by and/or caught-between hazards.  a) Worksite was located at 100 Military Ocean Terminal, Dock Yard, Bayonne, N.J. 07002                                     An employee was assigned to operate a Club Car 1500 Carryall to move materials from a staging area to the work area.  The employer failed to ensure that this employee was provided training on the safe operation of the vehicle prior to use, exposing employees to struck-by and/or caught-between hazards.  Violation occurred on or about August 14, 2020

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a) Worksite was located at 100 Military Ocean Terminal, Dock Yard, Bayonne, N.J. 07002  Employer did not report a work related hospitalization to OSHA within 24 hours.  Incident occurred on or about August 14, 2020
