29 CFR  1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.   At this workplace, an employee conducted painting operations at greater than six feet above a lower level and did not have a fall protection system in place.  The employee was exposed to fall hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    At this workplace, an employee conducted painting operations and the roof access ladder did not extend at least 3 feet above the building landing.  The employee was exposed to fall hazards.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.   At this workplace, an employee conducted painting operations  and was not trained to recognize the hazards related to ladders such as but not limited to:   Instance:  a.  The employee was not trained to recognized hazards of not placing the access ladder at least 3 feet above the building landing.     b. The employee was not trained to recognized hazards of positioning the access ladder on an active roadway to avoid struck-by hazards.
