29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about December 10, 2020, employee(s) conducting roofing on a 4:12 pitch roof was not protected from falling to the lower level. This exposed employee(s) to a fall hazard of approximately nine (9) feet, three(3) inches while working on the Northeast side of the structure.

29 CFR 1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed: (a) On or about December 10.2020, the Keller ten (10) foot step ladder leaned against the eave on the Southeast side of the structure was being used for access to and from the roof while not expanded and properly locked in place. This exposed employees to a fall hazard of nine (9) foot three (3) inches.
