29 CFR  1910.134(f)(2):Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator:  (a) Cumberland Crossings Retirement Community located at 1 Longsdorf Way, Carlisle, PA 17015:  On or about  November 16, 2020, the employer did not provide a fit test to all employees required to wear respirators.  The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus while providing care to suspected and confirmed positive COVID-19 residents.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.    a) Cumberland Crossing Retirement Community located at 1 Longsdorf Way, Carlisle, PA 17015:  On or about November 14, 2020, the employer did not record a work-related illness which resulted in a work restriction on the OSHA Form 300 or equivalent for the calendar year 2020.  b) Cumberland Crossing Retirement Community located at 1 Longsdorf Way, Carlisle, PA 17015:  On or about November 16, 2020, the employer did not record a work-related illness which resulted in a work restriction on the OSHA Form 300 or equivalent for the calendar year 2020.  c) Cumberland Crossing Retirement Community located at 1 Longsdorf Way, Carlisle, PA 17015:  On or about November 16, 2020, the employer did not record a work-related illness which resulted in a work restriction on the OSHA Form 300 or equivalent for the calendar year 2020.  d) Cumberland Crossing Retirement Community located at 1 Longsdorf Way, Carlisle, PA 17015:  On or about November 16, 2020, the employer did not record a work-related fatality on the OSHA Form 300 or equivalent for the calendar year 2020.
