29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  At jobsite located at 2362 Chateau Wichita, KS, the employer did not ensure that appropriate permission from the manufacturer was obtained by the user prior to using an attachment on the Sky Trak Telehandler, and did not ensure that markings for the capacity, operation and maintenance were changed accordingly. This condition exposes employees to struck-by and crushing hazards.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Employees engaged in residential framing activities at the jobsites located at 2362 Chateau Wichita, KS, were exposed to falls from approximately 12 feet from the working platform to the ground below, in that the aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
