29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment was not isolated from the energy source and rendered inoperative.  On or about Oct 9th, 2020, at the northwest side of the building, the employer did not provide employees with a an energy control program to safely operate and sanitize the 1200lb Double Sigma Arm cookie mixer. This exposed the employees to a caught-in hazard.

29 CFR  1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment.  On or about October 9, 2020, the employer did not ensure the Double Sigma Arm Cookie Mixer was shut down and locked out, exposing employees to severe injuries, and or amputations.
