29 CFR  1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  On or about December 22, 2020, in the engineering office by  the refreshments vending machine, an opening on the electric control wall panel (120/240 Vac. Max 1 phase 50/60 Hz) was not effectively closed.  This condition exposed employees to electric shock  hazards.

On or about December 22, 2020,  in the engineering office by  the refreshments vending machine an electrical panel box was observed missing its cover.  This condition exposed employees to electrical shock hazards.
