29 CFR  1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.   (a) Jobsite- 239 Blakey Court, Pittsburgh, PA: On or about December 20, 2020, an employee fell approximately 28 feet to the ground below while unloading tools and materials through a window opening.

29 CFR  1910.178(o)(1):The employer did not ensure that only stable or safely arranged loads were handled:  (a) Jobsite- 239 Blakey Court, Pittsburgh, PA: On or about December 20, 2020, employees operating the JLG Rough Terrain Forklift did not ensure the load was stable and safely arranged on the equipment before it was lifted in the air.
