29 CFR  1926.652(b):The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4):  At a jobsite located at The Woods at Creekside, Parkville, MO 64152, an employee was exposed to serious caught-in, struck-by and crushing hazards. The trench where the employee was working in was not adequately sloped in conformance with the available options.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a jobsite located at The Woods at Creekside, Parkville, MO 64152, an employee was exposed to serious fall, caught-in and crushed hazards. The portable ladder that was being used for access and egress into the trench did not have its side rails extended 3 feet (.9 m) above the upper landing surface being accessed.

29 CFR  1926.651(j)(2):Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  At a jobsite located at The Woods at Creekside, Parkville, MO 64152, employees were exposed to serious caught-in, struck-by and crushing hazards. An employee was working at the bottom of the trench. Another employee was operating the CAT 336F bucket excavator. The CAT 336F bucket excavator was positioned less than 2 feet (.61m) from the edge of the trench.    Holthouse Construction L. L. C. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.651(j)(2), which was contained in OSHA inspection number 1475482, citation number 1, item number 1 and was affirmed as a final order on 10/05/2020, with respect to a workplace located at Creekside Drive, Parkville, Missouri 64152.
