29 CFR  1910.28(b)(3)(ii):  The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems.  During the months of October and November 2020, employees were not protected from tripping into or stepping into or through an approximately 20 foot long by 12 inch wide and 16 inch deep hole that ran along the south wall of a hallway east of loading bays #1 and #2.
