29 CFR  1910.67(c)(2)(v): (v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift.   On or about January 1, 2021, and at times prior there to, an employee was not wearing personal fall arrest or travel restraint system with lanyard attached to the boom or basket while working from an aerial lift.  The employee was exposed to a fall hazard.

29 CFR  1910.30(a)(3)(iv):  The employer did not train each employee in the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer.   On or about January 1, 2021, and at times prior there to, employees were not trained in use of the correct personal fall arrest systems and equipment including but not limited to, proper hook-up, anchoring, tie-off techniques and methods to inspect and store equipment. Employees were exposed to electrical shock hazards while performing installation of fiber optic cable line.

29 CFR  1910.268(c): The employer did not provide training in the various precautions and safe practices described in this section and  insure that employees do not engage in the activities to which this section applies until such employees have received proper training in the various precautions and safe practices required by this section and did not certify that employees have been trained by preparing a certification record which includes the identity of the person trained, the signature of the employer or the person who conducted the training, and the date the training was completed:   On or about January 1, 2021 and at times prior there to, the employer did not provide training in precautions and safe practices nor did the employer certify that employees had been trained.  This exposes employees to hazards including but not limited to fall and electrical hazards.

29 CFR  1904.33(a):  The employer did not maintain the OSHA 300 log, the privacy case list, the annual summary  and the OSHA 301 Inident Report forms for five (5) years following the end of the calendar year that those records covered.   On or about January 1, 2021, and at times prior to,  OSHA 300 logs, privacy case list, annual summary and the OSHA 301 Incident Reports were not maintained.
