29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 12/02/2020, at 1440 S. Biscayne Point Rd. Miami, FL, employees engaged in demolition activities of a residential flat roof were exposed to a fall hazard of approximately 10 feet above ground level without a means of fall protection.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about 12/02/2020, at 1440 S. Biscayne Point Rd. Miami, FL, employees engaged in demolition activities of a residential flat roof at 10 feet above ground with unprotected skylights, exposing them to a fall hazard.
