29 CFR 1926.502(d)(23): Personal fall arrest systems were attached to guardrail systems or to hoists: (a) At the jobsite located near the intersection of Carmichael Road and Vine Street, Hudson, WI; While working from a steel I-beam at heights approximately 25 feet above ground level, the lanyard of an employee's personal fall arrest system was affixed to the adjacent guardrail of a JLG model 660SJ telescopic boom lift.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) At the jobsite located near the intersection of Carmichael Road and Vine Street, Hudson, WI; The side rails of a 27 foot Werner model D6228-2 Type IA fiberglass extension ladder positioned on the west side of a steel-framed building did not extend at least 3 feet above the landing surface when it was used by an employee to access and egress the first level of the structure.
