29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about January 6, 2021, at the front of the building, employees were accessing and egressing a roof using a ladder that did not extend at least 3 feet above the roof or landing surface.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) On or about January 6, 2021, while working on the roof, employees did not have adequate fall protection while performing roofing operations at a height of approximately 30 feet above the ground.
