29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At the jobsite, employees were exposed to fall hazards of approximately 9-feet 8-inches when they used two 16-foot aluminum extension ladders that were not set three feet above the roof landing at the right side rear of the house.   b) At the jobsite, employees were exposed to fall hazards of approximately 16-feet 9-inches when they used a 24-foot aluminum extension ladder that was not set three feet above the roof landing of the garage.   c) At the jobsite, employees were exposed to fall hazards of approximately 18-feet 2-inches when they used a 24-foot aluminum extension ladder that were not set three feet above the roof landing at the front of the house.
