29 CFR  1910.178(n)(4):  Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles and wherever vision was obstructed:  a.) Warehouse - On or about April 8, 2021, the employer failed to ensure that operators were required to slow down at cross aisles and wherever vision was obstructed.  An operator was observed driving a forklift at a high rate of speed and did not slow down while passing stacked pallets that obstructed the view of pedestrians.    b.)  Warehouse - On or about April 8, 2021, the employer failed to ensure that operators were required to slow down at cross aisles and wherever vision was obstructed.  An operator was observed operating an electric pallet jack at a high rate of speed and did not slow down in the aisle and where vision was obstructed.

29 CFR  1910.178(n)(11): Dockboard(s) or bridgeplate(s) were not properly secured before they were driven over;  a.) Warehouse, Docks 3 and 5 - On or about January 8, 2021,  the employer failed to ensure that each dock leveler was properly secured before they were driven over.  The dock levelers at docks 3 and  5 were elevated and not flat, thereby exposing employees to hazards associated with uneven surfaces.    b.) Warehouse, Dock 5 - On or about April 8, 2021, the employer failed to ensure that each dock leveler was properly secured before it was driven over.  The dock leveler at dock 5 was elevated and not flat, thereby exposing employees to hazards associated with uneven surfaces.
