OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, from amputation hazards from powdered metal press deficiencies:  A) Metaltech Incorporated, on or about January 08, 2021, at the address of 3547 Watson Highway Dubois, PA: Employees are exposed to amputation hazards from a lack of powder metal press inspections. Various powdered metal presses used for compacting powdered metal into parts had deficiencies due to a lack of inspections. These deficiencies included, but were not limited to: 1.	Missing and inadequate guarding of the point of operation 2.	Guards that are easily removed from machine while the interlock key is seated so that the machine will still operate while the guards are removed.  3.	Incomplete and/or inoperable safety devices, safety interlock keys were placed in the machine so it would operate and expose employees to point of operation hazards.   B) Metaltech Incorporated, on or about January 08, 2021, at the address of 3547 Watson Highway Dubois, PA: Employees are exposed to amputation hazards from a lack of powder metal press inspections. Various powdered metal presses used for compacting powdered metal into parts had deficiencies due to a lack of inspections. These deficiencies included, but were not limited to: 1. Press keys were left in the machine, which allowed employees to change the mode of the machine. Keys were not designated for authorized employees/supervisors. All employees could change the mode of the machine, machines including, M6, 15, 19,23

29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  A)	Metaltech Incorporated, on or about January 08, 2021, at the address of 3547 Watson Highway Dubois, PA: Employees are exposed to amputation hazards. Employees operating Forming Machines , 15, 19, 21,22,23,24 without the press being equipped with guarding over the point of operation. plexiglas guards were unattached laying beside the machine or open with the interlocks disconnected from the guard to operate the machine.   B) Metaltech Incorporated, on or about January 08, 2021, at the address of 3547 Watson Highway Dubois, PA: Employees are exposed to amputation hazards. Employees operating sizing machine 1 without the press being equipped with guarding over the point of operation. No guards were available for the machine, after an accident involving a point of operation finger laceration occurred six months prior.

Metaltech Incorporated on or about January 08, 2020 at the address of 3547 Watson Highway, Dubois, PA: Employees conduct servicing and maintenance work on machines and equipment that included but were not limited to powdered metal presses and sizing presses in that  (1) Specific energy control procedures were not documented/implemented (2) Authorized employee training was not conducted/documented (3) Affected Employee training was not conducted/documented (4) Periodic inspections were not conducted/documented.
