29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about January 11, 2021, the employer did not ensure employees, performing roof work activities, were in use of fall protection. This exposed the employees to a fall hazard greater than six (6) feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. On or about January 11, 2021, the employer did not ensure that employees, performing roofing work activities, were trained to recognize the fall hazards and the procedures to minimize those hazards. This exposed the employees to a fall hazard.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about January 11, 2021, the employer did not ensure the side rails of an extension ladder extended at least three feet above the landing. The employees used the ladder to access and exit the roof and were exposed to a fall hazard.
