29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Exterior of Building - Roof: Employees were exposed to eye injury while engaged in roofing activities without the use of eye or face protection, on or about 1/11/21.  b) Exterior of Building - Roof: Employees were exposed to eye injury while engaged in roofing activities without the use of eye or face protection, on or about 1/12/21.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a) Exterior of Building - Back Roof: Employees without any means of fall protection were exposed to fall hazards of approximately 16 feet while engaged in roofing activities, on or about 1/11/21.  b) Exterior of Building - Front Roof: Employees without any means of fall protection were exposed to fall hazards of approximately 25 feet while engaged in roofing activities, on or about 1/11/21.  c) Exterior of Building - Front Roof: Employees without any means of fall protection were exposed to fall hazards of approximately 15 feet while engaged in roofing activities, on or about 1/12/21.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Exterior of Building - Front Roof: Employees were exposed to approximately 15 feet fall hazard while utilizing an extension ladder that only extend approximately 18 inches over the working surface, on or about 1/11/21.
