Employees are not conducting pre-operational checks of these vehicles and the vehicles are being operated daily despite having a defect.   Powered industrial vehicles, to include but not limited to, the Caterpillar #9, serial #4EM06781, which had a burned out head light.  On or about January 13, 2021, the employer failed to ensure that powered industrial vehicles that were found to have a defect were not removed from service.  Employees are not conducting pre-operational checks of these vehicles and the vehicles are being operated daily despite having a defect.
