29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about January 13, 2012,  employees performing masonry construction activities were not protected from a fall while working from a metal framed scaffold, exposing employees to a falls between 11- 14 feet.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.178(l)(4):1910.178(l)(4)(iii):  An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.  On or about January 13, 2021, an employee was operating a powered industrial truck prior to being evaluated within three years, exposing employees to the hazards associated with operating a powered industrial truck.   No abatement documentation is required of this item.
