29 CFR  1926.403(b)(2):Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  On or about January 11, 2021, and at times prior thereto, three extension cords were inter-connected in order to achieve the length needed to supply power to a Seamer Machine, Makita Model DS4000, Serial Number 13958, exposing the employees to an electrical shock hazard.

29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables were used:  On or about January 11, 2021, employees were using an extension cord, to provide power to a Seamer Machine DS4000 (serial #13958), with the insulation worn down to the inner wiring of the flexible cord, exposing the employees to an electrical shock hazard.

29 CFR  1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  On or about January 11, 2021, employees were using an extension cord that was missing a ground pin on it's plug that was powering the Seamer Machine, Makita DS4000 (serial# 13958), exposing the employees to an electrical shock hazard.

29 CFR  1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  On or about January 11, 2021, and at times prior thereto, 3 roofers performing new sheet metal roofing activities were taking a measurement at the RS4 roof without using fall protection, exposing the roofers to a fall hazard of approximately 46 feet to the ground below.
