29 CFR  1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  a. On or about January 15, 2021, the employer failed to document through a written certification that the workplace hazard assessment for personal protective equipment had been performed. The employer did not have a document certified as the hazard assessment with specific required policies, did not have a document that identified the person who performed the hazard assessment and did not have a document to provide the date of the hazard assessment.
