29 CFR  1926.102(a)(1):  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On or about January 13, 2020, at the jobsite, the employer did not ensure that employees were in use of eye protection while using hammers and pneumatic nail guns to install roofing materials on the roof.   This exposed the employees to eye injury hazards.

29 CFR  1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   On or about January 13, 2021, at the jobsite, the employer did not ensure that employees installing roofing materials on a roof were anchoring to the roof with an anchor point and lanyard.  This exposed the employees to a fall hazard of more than six(6) feet to the ground below.
