29 CFR  1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident.  On or about December 30, 2020, the employer failed to report an employee death within 8 hours.  The fatality was reported on January 14, 2021.  This condition delayed the timely investigation of a workplace hazard.
