29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury.  a) Jobsite located at 900 Packer Avenue, Philadelphia, Uniform room, Carousel C: The employer failed to develop, Lock-out Tag-out program, which included but was not limited to energy control procedures and employee training for employees performing servicing and maintenance on machines such as the GBX-WD-1000 garment carousel system. Employee servicing garment carousel  was injured when unexpected energization of machine occurred.  Condition occurred on or about 8 JAN 2021.
