(a) On the rooftop of a residential structure, on or about January 11, 2021, no eye protection was in use by employees who were utilizing pneumatic nail guns to install roofing materials, resulting in an flying particle hazard to the eyes.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a) On the rooftop of a residential structure, on or about January 11, 2021, no fall arrest system or other forms of fall protection were in use by employees who were working while standing on the roof (pitch approximately 6:12), resulting in an approximately 11-foot fall hazard.
