29 CFR  1926.1053(b)(5)(i):Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:  8770 TX-151 Hwy: On or about January 13, 2021, and at times prior thereto, workers were exposed to a fall hazard from an extension ladder whose horizontal distance was less than one quarter of the working length of the ladder.
