29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about January 21, 2021, employees were exposed to a fall of approximately 13 feet while installing vinyl siding on a residential home located on SL 303 Dublin Way, Streetsboro, Ohio 44241.  An employee was working from a Ladder Jack Scaffold and the employer did not ensure that he was using fall protection.

On or about January 21, 2021, employees were exposed to a fall of approximately 13 feet while installing vinyl siding on a residential home located on SL 303 Dublin Way, Streetsboro, Ohio 44241.  An employee was working from a Ladder Jack Scaffold and the employer did not ensure that the two scaffold platforms were not bridged together.
