29 CFR 1910.23(b)(8): The employer did not ensure that ladders are used only for the purposes for which they were designed.  a) Storage Room, Youth Gymnasium: On January 19, 2021, and at times prior, the employer exposed employees to falls of up to 10 feet in height in that a Werner extension ladder permanently affixed to a wall of the storage room with screws and wooden planks was used by employees to access the mezzanine in the storage room to retrieve or store equipment and materials.

29 CFR 1910.23(b)(9): The employer did not ensure that ladders  are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury.  a) Storage Room, Youth Gymnasium: On January 19, 2021, and at times prior, the employer exposed employees to falls of up to 10 feet, in that the employer did not inspect the Werner extension ladder attached to the wall of the storage room with screws and wooden planks to ensure it was in safe condition for use to access the mezzanine.

29 CFR  1910.303(g)(2)(i):Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a) Storage Room, Youth Gymnasium: On January 21, 2021, and at times prior, the employer exposed employees to electrical hazards, in that a plastic sign was used as a cover for the electrical box located next to the metal extension ladder in the storage room that employees used to access that room's mezzanine.
