(a) Peninsula-McCabe United Methodist Church, 2200 Baynard Blvd., Wilmington, DE (church sanctuary) - On or about January 2, 2021, the employer failed to prohibit a Werner 32-foot 32FT Type IA Fiberglass D-Rung Extension Ladder (Model D6232-2) from being placed atop a Metaltech Jobsite Series 6' Baker ( Model I-CISCAS) mobile scaffold to increase the working level.

29 CFR 1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  (a) Peninsula-McCabe United Methodist Church, 2200 Baynard Blvd., Wilmington, DE (church sanctuary) - On or about January 2, 2021, the employer failed to train each employee who used a Metaltech Jobsite Series 6' Baker (Model I-CISCAS) mobile scaffold, resulting in the death of an employee who fell a distance of approximately sixteen feet off an extension ladder, set a top this scaffold's platform, to the floor.
