29 CFR  1910.1030(c)(1)(ii):The employer's Exposure Control Plan did not contain at least the elements in 29 CFR 1910.1030(c)(1)(ii)(A) through (c)(1)(ii)(C):  a) Facility -  On or about January 22, 2021, the  employer did not complete a written exposure control plan with required elements in accordance with this section.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR  1910.134(c)(2)(i):Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a) Facility: On or about January 25, 2021, employees were provided KN95 filtering facepiece respirators for voluntary use and  the employer did not provide employees with the information contained in  Appendix D of this section.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
