29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   a. 95 Grovers Mill Rd., Plainsboro, NJ - Community School Addition - Gym  The employer did not ensure that the working levels of scaffold were planked between the  front uprights and the guardrail supports.  Employees engaged in carpentry activities, installing steel framing studs, were observed working from welded tubular frame scaffolds that were not fully planked.  Violation observed on or about January 22, 2021

29 CFR  1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:   a. a. 95 Grovers Mill Rd., Plainsboro, NJ - Community School Addition - Gym  The employer did not ensure the scaffold was inspected prior to use.  Employees engaged in commercial framing activities worked from a two tier, welded tubular frame scaffold, with visible defects that included, but were not limited to, insufficient planking and missing guard rails when required, which had not been inspected by the competent person.   Violation observed on or about January 22, 2021

Employees engaged in carpentry activities, installing steel frame studs, used a scaffold without guardrails/end rails and were not utilizing any other means of fall protection. Employees engaged in carpentry activities used an A-frame ladder in the closed position as a means of access to the upper gym roof.  Violation observed on or about January 22, 2021
