29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At the jobsite located at 15698 165th St. Olathe KS, employee engaged in ground hand activities working below employees engaged in framing activities were exposed to struck-by hazards in that head protection was not utilized.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At jobsite located at 15698 165th St. Olathe KS, employees engaged in residential framing activities were exposed to falls from approximately 12.5 feet, in that the aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof.
