Great Falls, MT:  On or about January 25, 2021, and at times prior, employees were exposed to fall hazards while conducting roofing activities at heights of 11 feet 4 inches to the ground below without the use of fall protection.

Great Falls, MT:  On or about January 25, 2021, and at times prior, employees were exposed to fall hazards while being allowed to ride on the forks of a Genie GTH-5519 Rough Terrain Fork Lift to access and perform roofing operations on a residential home.
