29 CFR  1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to , 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    a) Homeland Vinyl Products Inc.- Production Facility located at 1701 Eden Road, Millville NJ 08332 - Production Line 8 Extruder: The employer's LO/TO procedure for the Milacron E80 HP Extruder, Serial # E54A0300027 was not specific as to include all energy sources including but not limited to: Hot water - which has separate valves, heat protection, how to release build up pressure, how to apply LO/TO hardware to plugs when more than one employee are conducting those operations. Authorized employees performed troubleshooting, unjamming, cleaning and minor maintenance operations without following specific LO/TO procedures to render all energy sources inoperative. Occurred on or about 10/ 27/2020.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:  a) Homeland Vinyl Products Inc.- Production Facility located at 1701 Eden Road, Millville NJ 08332 - Production Line 8 Extruder:  The employer did not ensure that ALL authorized employees were trained on all aspects of the LO/TO program and the LO/TO procedures for machines including but its not limited to:  Milacron E80 HP Extruder, Serial # E54A0300027, occurred on or about 10/27/2020.
