29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design:   a) Jobsite: On January 26, 2021 employees performed framing activities while elevated on a stack of sheathing material utilized as a work platform that was supported by a rough terrain forklift.  The scaffold platform was not designed and constructed by a qualified person in that the platform was not secured to the forklift. The employees were exposed to an approximate 11 foot fall to the ground below.  Note) Certification of corrective action is required for this item.

29 CFR  1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Employees performed framing activities while elevated on a stack of sheathing material utilized as a work platform that was supported by a rough terrain forklift. 29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used:  a) Jobsite: On January 26, 2021 and at times prior thereto employees performed framing activities while standing on a stack of sheathing material being utilized as a scaffold platform, supported by a rough terrain forklift, without a guardrail or other fall protection system in place.  The employees did not recognize or acknowledge the hazard and take action that would control or minimize the hazard.      b) Jobsite: On January 26, 2021, and at times prior thereto employees performed framing activities while standing on a stack of sheathing material being utilized as a scaffold platform, supported by a rough terrain forklift.  The platform was not securely attached to the forklift.  The employees did not recognize or acknowledge the hazard and take action that would control or minimize the hazard.   Note) Certification of corrective action IS required for this item.
