The scaffold platform was not designed and constructed by a qualified person in that the platform was not secured to the forklift. The employees did not recognize or acknowledge the hazard and take action that would control or minimize the hazard. b) Jobsite: On January 26, 2021, and at times prior thereto employees performed framing activities while standing on a stack of sheathing material being utilized as a scaffold platform, supported by a rough terrain forklift. The platform was not securely attached to the forklift. The employees did not recognize or acknowledge the hazard and take action that would control or minimize the hazard.
